Authorization to Use or Disclose Protected Health Information (PHI)

Down loadable privacy forms in multiple languages from the Directors Office. This authorization form may be used by you as a patient/client of the County in order to initiate a request to have PHI about you disclosed outside of the Health and Human Services Agency or between Programs with sensitive confidentiality requirements.

Documentary film series on transition-age youth

This 4-part documentary film, Understanding Transition Age Youth, was developed in collaboration with partners in the National Child Traumatic Stress Network (NCTSN) and ReMoved Films. This film will serve as a useful awareness raising and educational resource for young people, family members, and providers. Additional resources related to this film and our Center’s public awareness films website will be forthcoming soon.

Talking About Suicide with Friends and Peers

This resource provides information to support youth talking about suicide with friends and peers. This fact sheet includes what you can do, action words, skills for getting help, as well as myths and facts about how to help as a peer. This resource is most helpful for youth ages 12 and older.
